Beware of “Pneumonia Weather”!

I know that my Mom has many “mom-isms” (sayings that are specific to her), and growing up I believed these to be common knowledge or fact, which now I know that some were completely special to our family. But, to her credit, her declaration of “pneumonia weather” was based upon when I would get it (as I did as a child due to severe asthma) based upon different weather patterns and, by George, she was onto something! The number of respiratory tract infections (RTIs) going around right now are staggering. and for most of us in the US, it is “pneumonia weather”.  To be clear, while I am referring to it as “pneumonia weather”, as that is what my mom calls it, I am just referring to RTIs, which are the leading cause of people missing school and/or work annually.

It always chimes in the back of my head when I see people, who are real positive thinkers, wearing short-sleeved shirts in this cold weather in the end of March and beginning of April  because they feel like it should be warm, so they are pushing the limits, and really freezing in the high hopes that it will get warm, which it does in the middle of the day (with sun) then right back to cold. Now, it has nothing to do, per say, with being under dressed for the weather, it is more just a time when not only are humans confused but so is nature.  The allergens from the trees and flowers starting to bloom, mold spores from the old leaves from fall that are now seeing the light of day after our winter thaw, are all out in the air floating about, and people are getting outside more (which is great) to get more active as it is more tolerable to exercise outside now. But, that cooler air can still cause airways to become reactive, and add in the allergens (which no one ever thinks about this early in the season), add people starting to get in shape and exercise outside again, and you have the perfect setting for people to get RTIs, especially for those who are high risk: people with asthma (or other chronic lung diseases), seasonal allergy sufferers, smokers, the very young and old, those with weakened immune systems, and people with obesity.

*This “pneumonia weather” can also happen when the seasons change from summer to fall with leaves on the ground, mold spores in the air, and newer chilly weather outside.

What is a RTI?

A respiratory tract infection is any sort of virus that causes a response, in any part of the respiratory tract: nose, ears, throat, lungs.  So it can be an ear infection, a sore throat, a runny nose/sinus infection, bronchitis.   All of these are considered respiratory tract infections, as all (minus your ears) are a part of the system that our bodies use to breathe and/or to help filter out the air that we breathe before it gets into the lungs.  So, the more allergens in the air, the more we are breathing in, the higher the risk of a reaction, especially if you are someone prone to respiratory tract infections. RTIs if severe enough, along with a chronic disease and a lowered immune system, could lead to pneumonia.

How do I prevent it?

Follow the normal precautions that you take to prevent illness, such as eating a healthy diet, drinking lots of water, washing your hands frequently, and staying clear of people who are sick (says the nurse; but you all know what I mean, keep your distance if someone is actively showing signs of illness and coughing/sneezing everywhere, as best you can).  But on top of these measures, if you are someone who tends to suffer from seasonal allergies, start taking allergy medication early in the season, i.e. now, because even though the allergen levels aren’t through the roof, they are much more than we have been experiencing, so be aware of that and pay attention to your body. If you are just getting back into shape, ease into it and don’t over-train or over exert your body because on top of your body trying to recover from an increase in exercise and responding to that, if you are also exercising outdoors, you are adding all sorts of allergens too, which can make you more at risk to react to the allergens and get sick. There are some small claims that probiotics can help to prevent RTIs, so it is worth a try, if you are at risk!

If you do get an RTI, rest, drink fluids, eat a clean diet, and stay home from school/work so you don’t infect others.  While you feel miserable for a few days, only about 15% of them are actually bacterial infections, so they are usually viral and will clear up on their own.  If you have a high fever that won’t break, are not getting better, or have difficulty breathing, you should not hesitate to call your Healthcare Provider to get checked out….but don’t push for antibiotics, they will prescribe them if they are necessary :)

An Ease for Morning Sickness?

Morning sickness, which is kind of a misnomer as it can happen at any point during the day and may come and go sporadically, is very common during the first couple of months of pregnancy. Some people are able to dig deep and get through it because their symptoms are quite mild, whereas other people can have very severe nausea associated with their pregnancies.  For those with milder to moderate symptoms, a group of HCPs (Healthcare Practitioners) created Preggie Pops and Preggie Drops to help alleviate the symptoms of morning sickness.

These HCPs created the Preggie Pops using some alternative therapies based upon some accepted alternative therapies and some old wives tales. For example, they are made with a sour flavored base, as this has been handed down by generations of childbirth educators as a treatment for a sour stomach and to squelch a nauseous stomach. As well, they are made with mint and lavender which are aromatic and thought to promote relaxation (lavender) and aid in digestion (mint) along with ginger, which has long been known as a treatment for nausea, to help settle an irritated stomach lining. They are made with sugar, and sometimes nausea can start to occur in pregnancy when the blood sugar starts to get low, almost like a signal that you need to be eating some more because your body (and the baby) need nourishment, and because you are sucking on these pops/drops slowly, you are getting small amounts of sugar, instead of eating large quantities at one time, which totally makes sense!

Do they Work and are they Safe?

They have many testimonials stating that people have used them not only for morning sickness but also for nausea associated with surgery and other illnesses. But, they are a homeopathic treatment that is not FDA approved; they do not contain any drugs, but they are using homeopathic treatments that are untested. On their website they state that they are safe because they are all natural….there are many things that are all natural but they are not safe for your unborn child, or for you. So I urge people to take heed and caution, check with your HCP before using this product or any other for morning sickness/nausea associated with pregnancy, just to really make sure it is safe and you are under the care of an HCP, in case there are any side effects.

At the risk of your health, and your unborn child’s health, it is best to consult with an HCP before taking any sort of supplementation while pregnant.

Another reason to know the source of your food…

There have been numerous times that we have consumed products and then found out there were different additives or harmful substances present, which is usually due to a lack of adherence to regulations, or knowingly contaminate the product to make a larger profit.  This has recently become a rather hot issue is because of contaminated found in the UK, Ireland, and a few countries in the EU.

What was the contamination?

Some rather large quantities of commercially sold hamburger (“beef”) meat in Ireland was tested for DNA, per regular testing standards, and meat that was labelled as beef was found to actually have the DNA of pigs and horses present.  The meat was found to have come from suppliers in France and Ireland that have then supplied major retailers such as Tesco (for Americans, Tesco is like Stop & Shop, Winn Dixie, or Ralph’s, a pretty major chain of grocers) which produces its own foods.  Since the contaminated meats were found at one source, they were traced back to the sources and the issue was found to be much more widespread.  Tesco products such as Bolognese sauces, lasagna, and other products have been contaminated with horse meat.  Not only is the pig and horse meat contamination an ethical issue for many people, but there are also certain religious groups (such as Muslim or Jewish groups) that don’t eat porcine products due to beliefs, which makes this a huge issue.

One of the other issues is that the horse meat was found to have phenylbutazone (also known as bute) present in it.  Phenylbutazone is a non-steroidal anti inflammatory drug (NSAID) that is used for short-term pain control in animals.  It has been banned in both the US and the EU due to issues of aplastic anemia which results from suppressed bone marrow, so your white and red blood cells are not adequately produced, to properly clot or fight off infection. Even ingesting these meats, which were found to be up to 60% horse meat that was contaminated with phenylbutazone, which can be absorbed by humans and cause this bone marrow suppression and anemia.

Up to 16 countries in the UK and EU that have been affected by this meat contamination, not to mention that these suppliers also sent contaminated meat to large fast food retailers such as Burger King in the UK/EU, thus numerous people have been exposed to these different meats and the phenlybutazone. If you think you have been exposed, you should go to see your Healthcare Practitioner (HCP) and get assessed, but you should know that they are currently making the statement that ingesting these small amounts of the drugs through horse meat is presumed to not have an impact on your health, but it truly is unknown. If you feel unwell, and have been at risk, it is best to see your HCP and get checked out, especially if you have any other medical issues.  For a list of affected foods check here.

Another reason to eat local and support local agriculture when you can, you know what kind of meat and the source!

Have an annoying bump in your mouth? It may be a mucocele!

I bet many of you have had a mucocele and maybe didn’t know what it was, and by the time you think to figure out what it is, it’s gone! I tend to be a teeth grinder when I am stressed, much to the chagrin of my dentist, but every now and again, I will bite my inner lip, so I tend to be really sensitive to any sorts of bumps and lumps in my mouth because I usually just assumed I have done some damage during my midnight teeth mashing! Instead of the trauma caused by teeth grinding, you can get some small bumps on the tissues in your mouth that are little fluid filled bumps that may be a little painful, but more annoying than anything else, and that, my friend, is a mucocele!

What exactly is a mucocele?

Mucoceles are painless thin little cysts (fluid filled sacs which has clear fluid once popped but can look almost bluish inside your mouth) usually on the inside of the lips and/or cheeks, and are quite common. The can also occur on the roof of the mouth, the tongue, or on your gums. They can vary in size, and the larger they are, the more annoying they can be, but they usually do not cause any pain, but they can just be irritating depending where they are (especially if on the tongue).  They are thought to occur due to sucking the tissue from your lips/cheeks between the spaces of your teeth (it’s a working theory), so the size of the mucocele would depend on the size of the space between your teeth. Although, they are especially common around an sort of oral piercing sites, so I am not sure how that works with the teeth sucking theory!

How are they diagnosed and treated?

The good news is that Healthcare Practitioners (HCPs) can just visualize it to diagnose it, and once we diagnose it, I am sure you will recognize it in the future (to save yourself a co-pay and trip to the HCPs office!) That being said, sometimes they become large and will keep coming back, which would require them to be removed by an oral surgeon or a dentist (depending on size, placement, and their level of comfort). Often, smaller mucoceles will just rupture on their own, spontaneously, and then not come back. If they are big and causing a lot of irritation, an HCP may have to rupture it for you using a sterile needle (which sounds painful, but it’s not.) I do not suggest rupturing them with any sort of sharp object on your own, as you risk an infection….and oral infections are neither fun, nor easy to treat!

How can I prevent them?

There’s nothing you can do other than remove oral piercings that you may have that there are commonly mucoceles around, and trying not to suck on your teeth.  I am not sure if people really intend to suck on their teeth, I don’t think it’s a conscious thing….but if you do, try to stop!

The important things to know is that they are common, and if they become uncomfortable go to see your HCP to get them removed.  They are in no way detrimental to your health, but to make sure that if they do need to be ruptured or removed it is done under the care of an HCP to make sure it is as clean as possible and prevent the risk of infection. So, next time you might feel a little annoying bump in your mouth, you have a little more insight as to what it is!!

How to add calcium to your diet!

I know a lot of people that are lactose intolerant, or just tend to really dislike dairy, and want to add more calcium to their diet. Calcium adheres to the bones and helps to prevent osteoporosis. It can be a bit tricky to know how much you might need to eat of various foods to get the amount of calcium that you need each day. Calcium is also a bit of a tricky supplement to take because you need vitamin D to absorb the calcium, without vitamin D, you only actually absorb about 10-15% of the calcium you ingest.  Vitamin D can be taken in supplement form but the best way to get Vitamin D?  Spend 30 minutes in the sun, outside of the most intense sunlight times (10am-2pm) with only light sunscreen on (and it’s best absorbed without any on).

How much calcium do I need?

From the ages of 10 to 20 your body absorbs the most calcium to the bones, and slows significantly after the ages of 30. It is suggested that the Recommended Dietary Allowance (RDA)  for the ages of 9 to 18 is 1300mg, ages 18 to 50 is 1000mg, and ages 51+ is 1200mg per day. If you think that an 8oz serving of yogurt has around 400mg of calcium, and an 8oz glass of milk has around 350mg, cheese has about 300mg per 1.5oz serving, eating three servings of dairy, really help you to hit that RDA for calcium.  If you can’t tolerate dairy, or just don’t like it, fear not, there are other ways to get in enough calcium, you just need to pay attention to your diet.

How do I get my calcium on?

The good news is that we live in a world full of fortified foods, and soy, almond, and coconut milks all have around 300mg of calcium in an 8oz serving, as does Orange Juice fortified with calcium.  One serving of beans (1/2 cup) gives you around 100mg, 1/4cup of almonds (in any form) adds 80mg, and green leafy vegetables are chock full of calcium (1 cup of uncooked kale/spinach/etc has about 200mg).  And, if you are really into sardines, you are really in for a surprise, 8 medium-sized canned sardines has 370mg of calcium! And for all of you tofu lovers, 1/2 cup of tofu has 270mg of calcium.  Your after dinner treat of frozen yogurt? 100mg of calcium.  So pay attention to the foods you eat, most likely you are getting a good amount of calcium, and remember that most cereals and breads are fortified with calcium, so read the labels.

What else can I do?

Remember that your body can only really absorb a maximum of 500mg of calcium at a time, so if you are taking supplements, be aware of that!  Remember to get your sun for the Vitamin D to aid in absorption of calcium into your bones. Also doing low weight-bearing exercises, lifting low amounts of weights, ankle weights, helps to strengthen your muscles around your bones and keep the calcium in your bones instead of stripping off.  Too much calcium can cause some bad things, so make sure just to get the approximately the amount that you need! So get that calcium in and strengthen those bones!!

A Happy and Healthy New Year!

I hear a LOT of chatter about “my diet starts tomorrow!”, “I think I’m going to go gluten-free to lose some weight”, and my favorite “Just signed up for a new gym, so I know I’m going to go every day, and 2013 will be my year.” I appreciate all of the gusto, and that people decide to make a change, with a definitive date in mind (i.e. New Years day), and I REALLY want everyone to be successful, so why don’t we make a resolution we can stick to?  Perhaps, being healthier in 2013! There are so many things you can do to be healthier without making ridiculous requests of yourself that you know you won’t be able to keep.

How do I go about being healthier?

-Make a commitment to exercise: Try something new, something different, and something you aren’t committing to for a whole year! So don’t spend a ton of money at a gym, maybe get a pack of passes to a yoga and/or Pilates studio? Pole dancing? Spinning? Boot camp? Pick your poison; try something new, and stick to a two to three month commitment.  There’s no need to be married to it, if you like it, stick with it, if you don’t, you are not out a ton of money and you will find something new, but you are trying something new exercising, and you’re excited about it, which means you’re more apt to do it.  Paying a ton of money and signing a years contract to a gym does not mean you will actually be going there to run on that hamster wheel…right?

-Take the time for nutrition: Make a food journal, if you feel that you need to step up your diet and eat healthier, and figure out where your calories are coming from.  Don’t cheat yourself, but write down everything (including beverages) that you ingest during the day, and try to find ways to make it healthier, or cut calories.  Do you snack on candy at work? Bring trail mix (full of fiber, some sweetness from dried fruits, and protein from nuts) which will keep you satiated longer than candy and you will feel better, or just bring some protein in the form of nuts, yogurt, jerky, etc. if you bring your own snacks that you enjoy and are healthy, it will cut down on the amount of mindless snacking, that can add up in calories! Plus making your own food, you have more control over the ingredients and the amount of fats/sugars added, so your nutrition is literally in your own hands.

-Try to decrease refined sugars: It is difficult and definitely a commitment, but I am not a huge fan of people quitting things cold turkey, as it were. Watch the foods you are eating, and read nutrition labels, and try to cut out anything with extra sugars added and cut down on how much you add to your morning coffee (for example), or breakfast cereal.  Try Coconut palm sugar or agave, something that is not a chemical, with a low glycemic index that still makes your sweet tooth feel satiated!

-Commit to eat out LESS: Eating out or getting take out foods a lot, not only will cost a lot, but there are hidden fat and calories in take out food, that you wouldn’t add yourself.  If you don’t cook, maybe grab a friend and take a cooking class?  Or just teach yourself, as long as you make a commitment to learn to cook, you can do it…even if not burning toast is a feat for you, give yourself a pat on the back for getting it done! Some food items that seem like you have to buy them because they would be hard to make, are so easy to make and more delicious when you make them yourself, like: hummus and pita chips, salads, soups, etc..  So simple, and you can cut fat, calories, and extra sodium! I made a tomato basil bisque with 1/4 of the fat and calories that you would normally get at a restaurant- so easy, fast, and yumbos!

-Try not to obsess about numbers: I am the opposite of most people and I don’t even own a scale.  I judge my weight based off of how I look and feel in clothes.  I roughly know what I weigh, but I am never certain until I go for a check up and I get weighed! But, if you are someone who weighs themselves a few tips: don’t do it daily (especially for women, hormones can really mess with that number), but weigh yourself at the same time weekly (this will give you a better average), and don’t be so obsessed with ONE number that you want to weigh.  Just be healthy, and exercise, and your weigh will come down to what it should be; if you starve yourself or workout like bonkers to hit a certain number, once you hit it, you will most likely won’t stay there: you set yourself up for failure.

Truly making a commitment to yourself and bettering your health is the first decision that you make, from there on, it is so easy to stick to it: buy healthy foods (buy groceries primarily from the outer aisles of the store), stock up on healthy snacks and keep them around (if you have easy access to healthy foods you are more apt to eat them), and try some new exercises, grab a buddy and go! The only person that can impeded your commitment to a healthier you IS you, so make that decision, go for it fully, and take charge over your own body.  It will feel good to be healthy, and starting off with healthy choices makes you feel good and will lead to a chain reaction of healthy choices.  So here’s to 2013 and a healthier version of you!!!

Should I get my pollen on?

I have heard people suggest bee pollen to treat every single ailment! Seriously, you have a rash?  Bee pollen. Asthma?  Bee pollen. Digestive issues? Bee Pollen. Alcoholism? Yup, you guessed it! Bee pollen.  I was overhearing this conversation in public the other day, and I was just thinking, how can one supplement be able to cure everything and not everyone uses it? But, then I started to think maybe I had just overlooked it, which is completely possible; if I read about things, and I don’t somehow leave myself a reminder, it’s gone…or comes back at a random time, and I thought maybe this was one of those black holes of knowledge, so I decided to research it.

What is bee pollen?

Bee pollen is basically bunches of different pollens (considered the male sex cells of plants because it is needed to fertilize) that the worker bees go off to collect from various plants, trees, etc. and when it is brought back to the bee hive, it is mixed with various enzymes and bacteria present in the hive. It is basically packed into clumps in the hive.  The bees in the hive subside off of the bee pollen, as it has all the nutrients and vitamins they need to survive. Bee pollen is about 40% carbohydrate, 5% fat, and 5% protein, and filled with various amino acids, and nutrients (such as zinc, magnesium, etc) but the rest is all bee waste products (i.e. bee poop), fungus, bacteria, and insect parts. Yum! I know that makes you want to run out and grab some!

What can it help?

Bee pollen, as I stated before, purportedly is a fix all for all sorts of various maladies, such as:

- Colitis/ Digestive issues

- Arthritis (improves joint movements)

- Increases energy

- Decreases allergies (seasonal)

- Asthma

- Eczema

- Alcoholism

- Prevents bacterial infections

- Maintains wellness

- Weight loss

Does it actually work?

Despite some people being huge believers in Bee Pollen supplementation in your diet, and some say because of the carb/fat/protein ratio you can actually solely subside off of bee pollen alone, there are NO independent studies that show it actually heals or treats these issues.  Actually, there are numerous cases of people taking bee pollen supplements and having allergic reactions because they have a bee sensitivity OR a sensitivity to one of the plants pollens present in the bee pollen, and even going into anaphylaxis (a severe allergic reaction that leads to loss of airway, ability to exchange air, and can quickly lead to death without immediate treatment.) Also pregnant women and women that are breastfeeding should NOT take bee pollen due to risks of transference of the fungus/bacteria from the pollen to their babies.

If you take bee pollen, and it works for you, I am not going to tell you to do any differently, but I would urge people with many allergies or on medications from chronic conditions to talk to their HCP before starting to take bee pollen.  Also, there are some eco and ethical issues with bee pollen supplements, in that there is a world-wide decline of honey bees due to their sensitivity to various pesticides, and they do subside off of their pollen, so maybe us humans can find something else to use, and let the bee populations boost up before we go back dipping into their pollen pots?

Asthma: Medications and Alternative Treatments

As previously discussed, Asthma is a long-term disease that causes a narrowing of the airways due to an allergic or hypersensitive reaction.  The bronchus spasm, and make breathing very difficult.  There are numerous medications that can be taken either in pill form or inhaled, that are FDA  approved, or you can try some different alternative therapies: some include taking herbal supplements (which may or may not be helpful) and  some include diet, exercise, and breathing techniques.

Modern Medical Treatments:
Allergy medications- like Zyrtec or Claritin to prevent common allergies and possible flare-ups
Inhaled Corticosteroids- like Flovent or Pulmicort. These are inhaled medications that have low risks from long term use and take a few days to a week to build-up in your system and are used for long term prevention of symptoms.
Leukotriene Modifiers: Oral medications such as Singular that prevent asthma symptoms for 24 hours at a time. They can have some serious mental side effects like depression and hallucinations, so you need to be aware of any feelings of sadness that are new and unlike you.
Long Acting Beta Agonists (LABA): like Serevent are inhaled medications that open airways and prevent inflammation but they have been linked with severe asthma attacks if they are not taken along with an inhaled corticosteroid.
Combination Inhalers: a mixture of a LABA and corticosteroid, like Advair. They work well for long-term control of symptoms, but there is a risk of asthma attack because of the LABA involved.
Theophylline: a daily pill that helps to open the airways by relaxing the muscles around the airways. It is kind of a treatment of the past, and not used very frequently these days….it was a first line treatment years ago.

What are Alternative Treatments?
Breathing exercises: in which you stretch the volume of your lungs and learn to relax to take deep breaths, can help to decrease the amount you need medications. Yoga breathing techniques can help to learn to relax and the feelings involved with taking deep, relaxed breaths.
Diet: eating fresh fruits and veggies high in antioxidants can help improve lung functions and prevent asthma symptoms.  As well, Omega-3 Fatty Acids (found in fish, flax, etc) have been found to reduce inflammation throughout the body, including the lungs, so it can help to improve asthma symptoms as well.
Exercise: regular cardiovascular inducing workouts can hep to strengthen your lungs and improve their function.  Working out in cold weather can trigger asthma, or if outdoors during high allergy season, but in a gym with AC you can work out your lungs and body all at the same time!

Are there Herbal Supplements?
As always, they haven’t been extensively studied or FDA approved, so I can’t say I condone the use of herbal supplements but many people have reported decreased asthma symptoms when using these products, and they may benefit you: butterbur, dried Ivy, Ginko extract, Indian frankincence, reishi mushroom, and Choline.  It is best to take these under direction of your HCP or an herbal healer, so please discuss so they can talk to you about any interactions to look for and any reactions that may occur.

Does anything make Asthma worse?
Exposure to allergens: if you are allergic to something, and know it, it can make your asthma symptoms worse, or cause flare-ups so you should stay away from that allergen, or do your best to avoid it (like cigarette smoke, exhaust, or foods that cause allergic reactions.)
Cold weather: As stated above, it can cause constriction of the bronchioles and make asthma symptoms worse.
Foods:
- Anything high in saturated fats (meats, butters, etc) can make asthma symptoms worse
- Additives and preservatives can cause people to have reactions and common additives are artificial sweeteners and sulfites (found in cheeses, red wines, etc.)
- Milk products can cause an increase in mucus production and a thickening of the mucus, making asthma symptoms worse.

Asthma is different for everyone and the way we all react can be very different, so work with your HCP to see what best fits your lifestyle and your beliefs.  Work with your HCP on your Action Plan and take into account all aspects of alternative and medical therapies. You know your body best, so learn what your triggers are, and how to prevent them!

A little overhung??

Why do we get hangovers? I think people don’t really understand the metabolism of alcohol, because there are ways to prevent hangovers, and make that fun night that you may have imbibed too much, into a much softer morning wake up!

 How the body responds to alcohol metabolism:
Its how our bodies convert substances into other compounds that we can either use  or excrete . Oxidation is the process by which alcohol is metabolized in the blood. Obviously the alcohol is distributed throughout the body, including the brain, organs, and tissues after ingestion and during metabolization, and small amounts are excreted through your breath and urine in unmetabolized forms.
You have your favorite adult beverage, it goes into your intestines, then is absorbed into your blood stream, it is metabolized by various enzymes.  In the liver the enzyme that breaks down alcohol is called alcohol dehydrogenase (ADH) which turns alcohol into acetaldehyde, which is then broken down to carbon dioxide and water.  Alcohol is also broken down by an enzyme in the liver called P450IIE1, which can increase in number with chronic drinking (hence how people build up a “tolerance” for drinking….increase P450IIE1) and this is where most alcohol is metabolized.

The liver can only metabolize a certain amount of alcohol per hour, no matter if you are a virgin drinker or if you have your sea legs.  That being said, the ability to metabolize alcohol is dependent upon the number of metabolizing enzymes, which varies in each individual and can be genetic (i.e. over half of all east Asians have low numbers of ADH enzymes so they tend to get drunk easily, have wicked hangovers, and get flushed with drinking which is due to a fast heart rate which is how their bodies respond to metabolism of alcohol.) Usually, once you have a drink, your blood alcohol level peaks within 30-45 minutes.

What other factors influence alcohol Metabolism?
Food: The higher the dietary fat content in the stomach/GI tract increases the amount of time it takes for the alcohol to get absorbed into the blood stream.  One study showed that people who ate a meal of fat, carbs, and protein absorbed alcohol 3 times more quickly than those who drank on an empty stomach.
Gender: Unfortunately, women absorb alcohol differently than men.  Us ladies will have a higher Blood Alcohol Content (BAC) than men drinking the same amounts, and we are more susceptible to alcoholic liver disease, heart muscle damage, and brain damage.  It is presumed that the difference is due to women having a smaller amount of body water and we may have a lower amount of ADH (never been proven but highly likely based on numerous studies).
Effects of Alcohol:
Body Weight: interestingly, Women who drink (moderately) tend to have significantly lower body weights than those who don’t drink at all.  Men, there is no weight difference, but if non drinkers add alcohol to their diet, they don’t tend to gain any weight.  Chronic alcoholics tend to lose weight because they substitute alcohol for food….
Sex Hormones: Chronic alcohol intake causes significant hormonal imbalances; the liver metabolizes estrogen into testosterone, but when damaged, it is unable to do that, so you can have decreased sperm production.  In men who consume 220 grams of alcohol daily for 4 weeks, testosterone levels decreased after 5 days and kept dropping throughout the study period.  Also, chronic alcohol intake in men can lead to testicular atrophy (getting smaller) and the growth of male breast tissue. In women, we can create estradiol, which increases bone density and decreases the risk of cardiac disease (whoo hoo!!)
Medications: Tylenol (paracetamol, acetaminophen) can be toxic in the liver in large doses by itself, but add alcohol and it is a HUGE problem that can lead to immediate liver failure.  Like 4-5 extra strength pills in the course of the day with someone who is drinking can cause death. Other drugs such as pepcid (famotidine), ranitidine (zantac), and cimentidine (tagamet) can cause alcohol to be metabolized slower for reasons truly unknown at this point.
What can also cause increased rate of metabolism?  
The only thing known to increase the rate of metabolism of alcohol is fructose!! Consuming 100 grams of fructose has been shown to increase alcohol metabolism by 80%!  So, not helpful for people like me that drink vodka sodas, but great for the rum and coke (not diet coke) drinkers of the world!!  I guess I need to go back to my gin and Q-Tonic beverages OR maybe a handful of skittles after every drink???
So, the best way to prevent a hangover is to eat a meal with fat, protein and carbs (cheeseburger anyone?), increase your water (before bed chug a coconut water if you have one, or just regular water) because we all know the metabolism process dehydrates our bodies (because of the effects of alcohol), and increase your sugar intake….high fructose corn syrup does count!!  So seriously, a great idea for a bar is to hand each patron a bag of candy with each drink!!  Actually, one of the reasons why “fruity” drinks tend to make you less drunk :) hmmmm…..  BUT I must say, carbonation supposedly increases your alcohol absorption, so maybe a Harvey Wallbanger is the way to go…and try to stick with the same kind of alcohol.  Really, I think we all knew these tips except for the sugar.
So, ladies apparently drinking is good for you?!?!  And men, if you are looking to be more manly, you may want to lay off the booze for a bit….

This is a little hard to talk about: Erectile Dysfunction

Many people are unclear as to what Erectile Dysfunction (ED) actually is, despite those really ridiculous Cialis and Viagra commercials on TV (which can be horribly awkward to watch in mixed company at times!!) ED affects between 15 and 30 million men in the US and approximately 22.3% of all office visits to HCP’s are for ED.  It is something that many men are embarrassed about and feel unable to speak to their partner about, and it can cause huge rifts in relationships. It is important to know that signs and talk to your partner about it, it is a physical issue that is normal, and totally fixable, so go to the doctor together and make it a couple problem, and don’t let your partner suffer and feel badly alone!

What exactly is the definition of ED?

It’s the inability to maintain an erection that is firm enough to have sex, on an ongoing basis, along with trouble getting an erection, difficulty maintaining an erection, and/or a reduced sexual desire.  If you have any of these symptoms, especially if you have diabetes or heart disease, you should go see your HCP and talk to them about it, your options, and to find out what might be going on with your body. Don’t let it get too far that your partner might get upset or think that they are the one causing the problem…be open about it and go to see your HCP.

There are SO MANY possible causes of ED, in which some are physical, mental, and a combination of the two.  Some physical causes are: heart disease, atherosclerosis, high blood pressure, diabetes, obesity, metabolic syndrome, Parkinson’s disease, Multiple sclerosis, low testosterone, Peyronies disease (a build up of scar tissue in the penis), tobacco use, alcohol (alcoholism), substance abuse, pelvic/spinal cord surgery, enlarged prostate, and prostate cancer.  So, obviously it goes from being something that is easily fixed, to things that are more serious. As far as psychological issues: stress, depression, anxiety, fatigue, and relationship issues can all cause ED. Another psychological reason that many men have a form of ED, in this current society, is due to the inability to get their partner pregnant; we have a society wrought with infertility and it can case serious relationship problems on many levels. As well, many medications that your HCP might put you on for various health issues, can also cause ED.

Before you go to your HCP appt:

Write down symptoms that you have had even including things that you might think unrelated to ED.  Also, write down any life changes going on, including anything that might be causing relationship stress.  An update list of current prescribed and over the counter medications are always important to bring to every appointment, but especially to an appointment regarding ED.  Try to bring your partner to your appt and both of you talk before you go and both write down list of questions both separately and together.

What might happen at/after your appt?

A physical exam, which is standard at all appointments, but your HCP will inspect your penis, testicles, and check for nerve feelings.  Most likely you will have blood drawn to assess your risk of heart disease, diabetes, cholesterol, and testosterone levels.  A urinalysis may be performed to also look at your risk of diabetes, etc.  An ultrasound may be done to look at blood flow to your penis, it uses a plastic probe on the outside of your penis to project a video of the current blood flow (a non invasive exam). They also might have you go home and perform an overnight erection tests; many men have erections at night during sleep, and have no idea that they had an erection, if the test is positive, then the reason you have ED is psychological.  What they will ask you to do is to put a piece of tape that they will give you (it is easily split) around your penis, and if you wake up in the morning and the tape it split open, you’ve had an erection.

How do we treat it?

You can take oral pills like Viagra, Cialis, and levitra which are medications that enhance your bodies natural nitric oxide levels which helps muscles relax and allows more blood flow to your penis, allowing for a stronger erection.  You may need to change doses and change times that you take before getting an erection.   You need to talk to your HCP to take these drugs if you also take any nitrate drugs like nitroglycerin, Imdur, or Isosorbide because you can have too much of a build up of nitrates in your blood.  Also be careful with blood thinners and alpha blockers (for enlarged prostate).  Your HCP will have to discuss using medications if you have had a stroke, have either low blood pressure or uncontrolled high blood pressure, heart disease or heart failure, and/or uncontrolled diabetes.
Other medication options:
Alprostadil injections: a small needle is inserted in the base or side of the penis and you inject the medication, due to the small size of the needle there is usually little to no pain, and you can get an erection within 5-20 minutes and the erection usually lasts around an hour.
Alprostadil penis suppository: Using a special applicator, you insert a small suppository 2 inches up the inside of the urethra. The side effects can be pain, bleeding from the urethra, dizziness, and can cause a fibrous build up inside your penis.
Testosterone replacement: if you have low testosterone levels, oral supplements can help to increase your levels and ability to maintain/obtain an erection.
Other options:
Penis pumps: a hallow tube with a hand or battery-powered pump, you put your penis inside, pump out the extra air, which causes blood to flow to the penis and cause an erection, you need to put a tension ring on the base of your penis, then remove the pump.  The erection usually lasts long enough to have sex, then remove the tension ring.
Penile implants: implants are surgically places on both sides of the penis, either inflatable or semi rigid rods.  The inflatable implants, are inflated before sexual intercourse to help obtain an erection and the semi rigid rods allow for a firm but flexible penis at all times. Due to it being surgical in nature, there is a risk of infection, and usually a later option for ED.
Blood vessel surgery: at times a leaking vessel can cause a decrease in blood flow to the penis, a simple surgical procedure may be able to fix that problem.
Psychological counseling: if the problems are psychological in nature, counseling can help to talk about the problems and may help to decrease ED issues.

How can you fix it with lifestyle changes?

If you use tobacco, quit. Lose weight to get to a healthy level, exercise regularly, get treatment for alcohol and drug abuse, and work through relationship issues with your partner and with the help of a professional.

Alternative medicine:
Acupuncture is said to help with ED and relieve stress and anxiety, so it can help with some psychological problems.

Due to all of the possible causes, it is not only the first sign that something serious might be going on, but it also is something that might be fixed simply and easily, so I really encourage seeing an HCP early to talk about it.  Be open and honest with yourself, your partner, and your HCP.  ED is not something to be embarrassed about or something that you should allow to ruin a relationship.  If you notice any of these problems with yourself or your partner, please talk about it and go see you HCP ASAP!!
